Please download the Registration forms and complete in full. Swimmers registering prior to April 30^th will receive a Team T-shirt ($16 value) with their registration.  Swimmers registering after this date, will need to purchase a team T-shirt from the Club Store if they want a team shirt.

You may pay online using Master Card, Discover or Visa on the LJ Swim Payment Page.  Just download the following 4 forms and complete in full.  Then e-mail all 4 forms to ClubManager@ljclub.com or fax to 336-450-4000.
